How do you establish peer review groups?

In the Hacker handbooks, Nancy Sommers and I have been committed to establishing strategies and guidance for peer reviewers and for teachers who use peer review. We're curious, though: If you establish peer review groups in your class, how do you do so? And when? Do you group students randomly at the beginning of the semester? After a brief diagnostic essay? Do you group students by skill level? By major? By topic? Do you keep students in the same group throughout the semester--or do you recompose the groups one or more times in a semester? What do you feel are the benefits of the way in which you operate? And are you thinking through any new ideas to try?
